Welcome to Mill Pharm Ltd pharmacists’ model publication scheme. This is the model publication scheme for Mill Pharm Ltd trading as Mill Pharmacy.
Under the Freedom of Information Act 2000 all public authorities are required to have and operate a publication scheme approved by the Information Commissioner. Those providing pharmaceutical services under contract to the NHS in England, Wales and Northern Ireland are public authorities in respect of information relating to those services.
It is the intention of the Information Commissioner that all public authorities should adopt and operate the one model scheme that has been approved. This is a very general scheme based on the principal that all public authorities need to recognize the public interest in the transparency of the services provided for and paid for by the general public. It is a commitment to make information easily available to the public.
Note: The scheme is only for information held as a public authority and does not include any information that is held for other purposes or would be exempt from release. Our commitment to publish information excludes any information which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are, for example, the protection of commercial interests and personal information under the Data Protection Act 1998. This applies to all Classes within the Publication Scheme.
The scheme requires three documents to be considered:
• The model scheme itself;
• Guidance on adopting and operating the scheme; and,
• a guide provided by the public authority indicating what information will be provided, how it will be provided and whether any charge will be made for its provision.
Fees should be requested only where this is done in accordance with our guidance.

Introduction
The publication scheme is a complete guide to the information routinely made available to the public by Mill Pharm Ltd. It is a description of the information about our pharmacists and pharmacy which we make publicly available. It will be reviewed at regular intervals and we will monitor its effectiveness.
How much does it cost?
The publications are all free unless otherwise indicated within each class.
How the information is made available
The information within each class is available from the Superintendent Pharmacist and where indicated within each class on our website. The day to day running of the scheme and the overall responsibility of the scheme also lies with the Superintendent Pharmacist.
Your rights to information
In addition to accessing the information identified in the Publication Scheme, you are entitled to request information about Mill Pharm Ltd under the NHS Openness Code 1995.
The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how decisions are made.
From 1 January 2005 it obliges the pharmacy company to respond to requests about information that it holds about NHS Pharmaceutical Services that is recorded in any format and it creates a right of access to that information. These rights are subject to some exemptions which have to be taken into consideration before deciding what information can be release
Under the Data Protection Act 1998 you are also entitled to access your patient medication records or any other personal information held about you and you can contact the Superintendent Pharmacist to do this.

Our standards and the range of services we provide under contract to Suffolk PCT.
Every pharmacist must comply with the legal requirements on the sale and supply of medicinal products and comply with the professional standards set down in Medicines, Ethics and practice published by the Royal Pharmaceutical Society of Great Britain. For some services pharmacists and staff are required to attend training courses and sign service agreements with the PCT.

It is a requirement of the practice allowance that we receive from the PCT that we have a practice leaflet available dealing all our services. This is available from Mill Pharmacy and a copy can be vied/downloaded from our website.
Our inclusion on the List of Suffolk PCT Pharmaceutical List is subject to Terms of Service. The Terms of Service for pharmacists and chemists can be obtained from Suffolk PCT.
Mill Pharmacy keeps computerised patient medication records and is registered under the Data Protection Act.

Funding Details and Charging Policies
The prescription charge from 1 April 2014 is £8.05.Some patients get free prescriptions under certain circumstances – please ask for leaflet HC11 (www.dh.gov.uk/en/Healthcare/Medicinespharmacyandindustry/Prescriptions/NHScosts/index.htm)
Anyone who has to pay and gets more than 5 items in 4 months or 14 in 12 months could save money with a prescription prepayment certificate (PPC). You can also call 0845 850 0030 to buy one over the phone or to get more details – please ask for application form FP95 (or see (www.dh.gov.uk/en/Healthcare/Medicinespharmacyandindustry/Prescriptions/NHScosts/index.htm)
Since 1 April 2014 a 3 month prepayment certificate costs £29.10 and a 12 month one costs £104.00. These charges are set annually by the Department of Health.
The prescription charge is a tax paid directly to the NHS. It is not received by the pharmacist and does not form part of any remuneration to the pharmacy for NHS pharmaceutical services. The average ingredient cost of a dispensed item is £9.17 and pharmacies receive a dispensing fee for medicines of 90 pence per item (figures correct November 2009). Further information on the cost of drugs and appliances can be found in the Drug Tariff available online at http://www.ppa.org.uk/ or a paper edition of the Drug Tariff can be obtained from the Stationary Office of the Prescription Pricing Department. The Drug Tariff is updated monthly by the Prescription Pricing Department.
Some information in this class may not be released because it is confidential or commercial information or its release may be prejudicial to the conduct of the pharmacy’s affairs.
Complaints: policies, procedures and contacts for complaints
The complaints policy about NHS services provided by Mill Pharm Ltd is in accordance with NHS regulations and is available on request from the Superintendent Pharmacist. If you wish to make a complaint contact the Superintendent Pharmacist. The complaints policy is in line with the Pharmaceutical Services Regulations and the NHS Complaints Policy. If you are dissatisfied with the result of our investigation you can complain to the Health Service Ombudsman www.ombudsman.org.uk
You may also seek advice from the local Patient Advice and Liaison Service (PALS). PALS are not part of the complaints procedure itself but they might be able to resolve your concerns informally or they can tell you more about the complaints procedure and independent complaints advocacy services.
